We  are here to assist you with as many of your healthcare needs as possible! But we need a little help from you to get you  started.

To be Eligible and Registered to receive treatment at Fayette CARE Clinic, ALL patients must meet the following:

• Be a resident of Fayette County, GA
• Be at least 18 years of age
• Be without any form of health care insurance (no private insurance, Medicare, Medicaid or veteran's benefits)
• Provide a valid and unexpired photo I.D. with current address.
• Have Income that is 200% or below the Federal Poverty Guidelines.
• Provide proof of household income from the past 30 consecutive days, plus any unearned income, which may include:
  • Child Support
  • Social Security (Disability, Retirement, etc.)
  • Unemployment
  • Workers Compensation
  • Alimony
• Provide a proof of residency document (utility bill [electricity, water, gas] from the previous month with patient's name and address listed, ID must have current address.). NOTE: Cell phone bills and leases do not prove residency.
• For patients living with someone else, a Notarized Affidavit is required from the homeowner. Homeowner must state that the patient lives with them and provide information about financial support (if any) the patient receives from them. A proof of residency document showing the name and address of the homeowner is also required.
• If not receiving any wages, you will need a "No Wage Letter" for yourself  (and your spouse, if applicable) from the Georgia Department of Labor .
• Registration hours are Monday, Wednesday, and Thursday 9 am - 3 pm.
